West-Hill Health Primary Care Network
Our Practice is part of the West Hill Health Primary Care Network (PCN).
There are 1,250 PCNs across England based on GP registered patient lists. PCNs bring together GP Practices, community, mental health, social care, pharmacy, hospital and voluntary services. West Hill Health PCN’s vision is to provide accessible and high-quality healthcare while maximising NHS resources and reducing barriers.
PCNs have been established so that they secure the right workforce balance across the local neighbourhood to support GP practices in delivering good quality primary medical services.
West-Hill Health PCN includes the following practices:
- Holland Park Surgery
- Pembridge Villas Surgery
- Portobello Surgery
- Lancaster Gate Surgery
- Garway Road Practice
- Grand Union Medical Centre
- Goldbourne Medical Centre
- North Kensington medical Centre
The PCN has a population of 80,000 patients and our aim is to shape the services we deliver to the needs of our practice populations using a well-trained workforce to deliver good quality primary core services.
West Hill Health has achieved the highest patient satisfaction with their GP practices in comparison to the other PCNs in North West London in the MORI Poll for year 2023 and 2024.
